If you are new to the game, I would highly recommend you creating a small world to begin with. If you have played the game before and are use to the mechanics than you could go with a medium or large world. Though I still recommend creating a small world, the others I feel are more for multiplayer purposes.
If you are a new player than definitely choose a normal world. Expert is much hard, especially if you don't fully know what you are doing.
Pay attention to your loading bar while the world is being created. This will help you determine if your world if crimson or corruption. Red being crimson and purple being corruption. Depending on the biome you get the boss fight will be different though I believe equally challenging.
